DO NOT LEAVE YOUR MANITOBA HEALTH CARD NUMBER OR ANY PERSONAL HEALTH INFORMATION OVER VOICEMAIL.

When using NP Care At Homes services, we may collect certain information including:

• Patient demographics such as full name, address, telephone number and email address

• Payment information such as billing information, credit card numbers and bank account details pending on how you chose to cover the costs associated with your appointment

We also understand that there may be times when it becomes necessary for us to disclose some of a patient's confidential information in order for them to receive the best possible care. In these cases, we will only do so if we have received explicit permission/consent from the patient or legal guardian or if we believe that such disclosure is necessary due to an overriding public interest or legal requirement. When we share this confidential information with other healthcare providers, we take extra steps to make sure it is kept secure and used only for its intended purpose.
