Our services are available by appointment only allowing you to know exactly when you will be seen. Our goal is to provide same day or next day appointments. We make care possible for individuals without a car or means of transportation, for those without childcare, those too ill to drive or when travel is hazardous. Nurse Practitioner & Founder: Sabrina Mlinar Singh, BN MN NP
My name is Sabrina Mlinar Singh, BN MN NP.
I graduated with my Bachelor of Nursing degree from The University of Manitoba in 2013. I began my career practicing as a registered nurse in the regional emergency medicine program as well as acute adult medicine, both within The Health Sciences Center. During this time, I was exposed to the many gaps and barriers in the delivery of care to our community members, motivating me to want to help bridge these gaps and improve access to equitable healthcare.
I am an autonomous, accountable and self-motivated individual who is dedicated to my career and wanted the opportunity to expand my scope of practise by expanding my therapeutic knowledge and research to provide a clinical focus of nursing care. In 2018, I graduated from Graduate Studies, College of Nursing, Rady Faculty of Health Sciences at the University of Manitoba obtaining my masters in the Nurse Practitioner (NP) stream. Taking on this challenge has allowed me to solidify my leadership roles as I practise as an advanced practitioner. I enjoy providing holistic care from a clinical perspective while I aid in diagnosis, treatment, rehabilitation as well as palliation.
Functioning as an autonomous practitioner has taught me to become acquainted with each patient’s individual needs as I include their support system in a collaborative approach to care. Allowing patients an opportunity to be active members in their own health brings patients, families and clinical teams together to improve patient safety and health outcomes. At NP Care At Home that is exactly my goal. I want patients to feel respected, perceived and feel as though they have control over their own health. Providing patient-family centred care allows me to look out for my patients best interests while ensuring the best health outcomes.

Professional Experience: Upon completion of my undergraduate degree, I worked in the provinces largest territory trauma centre, The Health Sciences Centre, in both acute adult medicine, high observation, as well as the adult emergency room (ER).
After obtaining my masters degree, I began my NP career in the acute care program working in the Winnipeg Regional Health ER Program. During this time I provided acute episodic care to individuals requiring both urgent and emergency medical attention. In 2020, I decided to move into primary care as well as walk-in connected care to allow for a better family life balance. During this time, I have been able to develop trustworthy relationships with colleagues on the multidisciplinary team as well as my patients all while allowing me the opportunity to ensure continuity of care. I feel like this is essential when caring for individuals with complex primary care needs.
Being a front line worker in the midst of a pandemic with concern regarding ongoing nursing shortages, provided me an opportunity to support seniors in our province who required prompt, frequent medical assessments. I voluntarily took on the role of being a primary care provider to seniors living within the personal care homes in my surrounding communities. I was able to provide direct patient care to individuals requiring regular health assessments and management of both acute covid as well as management of chronic co-morbid conditions. During this time I developed rapport and trustworthy relationships with both my patients and their' families. As I begin my journey with NP Care At Home, I would like to continue to support seniors within our province as I have an appreciation and compassion for caring for their needs and ensuring that they are enabled to a live a longer, higher quality of life. The benefits of NP Care At Home is that I am able to meet their needs with my fully mobile clinic, providing care to them in the comfort of their' own residence.
In my walk-in connected care experience I have been able to provide health promotion and disease prevention education to individuals of all ages. During this time I have recognized the need for portable care for our communities including our transient community members which is why I am happy to service all the Winnipeg areas and surrounding communities.
During my training as a graduate student I was also fortunate to have spent time learning about the complex needs of trans-health individuals living within our province. Since that time I have continued to provide culturally sensitive care to these community members within my primary care practice and I have taken time to do research and focus on their needs as I continue to expand my knowledge and understanding of gender incongruence.
I want my clients to know that NP Care At Home is a safe space for all and each persons needs are evaluated on an individual basis as I am here to optimize health outcomes while working together to ensure the best interest of all my clients. The benefits of being a mobile clinic is that we can service your needs in the comfort of your own space, whether it is at home or work or via virtual telemedicine.

Biography: I am very passionate about my family including my spouse and two children. During my spare time, I enjoy spending time with my family and friends and traveling learning about new cultures.

The color's of the brand highlight each of my family members birth stones giving it a special meaning.
